Accepting New Patients in Texas, Oregon, and Washington State
If you prefer to pay out of pocket or don’t have insurance coverage:

A fixed amount you pay for each appointment (e.g., $30 per visit).
The amount you pay out-of-pocket before your insurance starts covering services. Once you meet your deductible, your insurance kicks in.
After you’ve met your deductible, you might pay a percentage of the cost (e.g., you pay 20%, insurance pays 80%).
The most you’ll pay in a year. Once you hit this amount, insurance covers 100% of covered services.
In-network providers have negotiated rates with your insurance (usually lower cost to you). Out-of-network providers may cost more, but you might still get partial reimbursement.
I accept the follow insurance plans:
Note: While I am in-network with these insurance companies, coverage can vary by your specific plan. I recommend verifying your benefits before scheduling.

Don’t see your insurance listed? I can provide a superbill for you to submit to your insurance for potential out-of-network reimbursement. Since coverage varies by plan, I recommend contacting your insurance provider to learn more about your out-of-network benefits
